Breastfeeding can be a time of great joy, but in the very beginning, women can face several challenges. One of these is what to do when your milk comes in. Here are five things you can do to help alleviate the swelling and pain in your breasts.
- Nurse your baby - It might seem like you just finished nursing the baby, but nurse him again anyway. Babies need to nurse early and often and they are much better at emptying the breast than even the highest grade breast pump. In fact, do not use a breast pump because your breasts are learning your baby's supply and demand and pumping will make your body think there are two babies needing food, causing you to make even more milk.
- Use ice for the swelling - Know that most of the fullness is tissue swelling, not milk. When the milk comes in, the breast is learning a new skill and the blood engorges the tissues, causing that tight extreme fullness. Just as with any other swollen body part, the breasts do well with ice on them. You can put a few wet washcloths in the freezer, laid flat so when you use one on each breast, they will conform to your body. Many people find, however, that using frozen vegetables work even better and last longer. Twenty minutes out of each hour is a good schedule to keep. Ice goes on after a nursing.
- Use wet heat - Right before you breastfeed your baby, it is very helpful to use wet heat. Piping hot wet washcloths are perfect for the job. Some women keep several washcloths in a crock pot filled with water so they do not have to keep running the water in the sink. Heat not only soothes the swollen breasts, but also softens the tissue to prepare for nursing. Breasts can be so swollen the nipples are flat, not allowing the baby to latch on properly, if at all. When the breasts get softer from the wet heat, you can express some milk out, softening the nipple enough for the baby to grasp it correctly. You can put wet heat on any time you do not have the ice on your breasts. One more way to utilize wet heat is to get in the shower and allow the hot water to soothe and press down on your sore breasts. Some women cannot tolerate the drops directly on the breasts themselves, so keep the spray up higher on the chest. Leaning over allows the breasts to leak out, softening the nipple even more when expressing is too painful.
- Use cabbage leaves - Cabbage leaves have a wonderful capacity for relieving engorgement. Some women put them in the freezer to make them very cold, whereas others will put the ice or hot packs on top of the leaves; do whichever feels best to you... it is the cabbage leaves themselves that relieve the engorgement. They also have the capacity for lowering the milk supply, so only use the leaves for 20-30 minutes 2-3 times a day.
- Take an NSAID - With permission from your care provider, take an NSAID such as ibuprofen. This helps to relieve engorgement as well as relieves pain.
- Hang in there! - Engorgement only lasts for a couple three days and if you can do these helpful hints, you will pass through the experience in a much more comfortable way than if you did nothing. The swelling would still go down, but it could take longer. Keep nursing even when you are very full and the supply and demand of your milk supply will quickly regulate itself.
- While breastfeeding has some early obstacles, they can be overcome with information and support. Call your local La Leche League leader or another successfully breastfeeding mother. Ask your family to be patient as you tell them how much it hurts. The answer does not have to be "Give the baby a bottle." Even if you have no one around, keep this article close to you when your milk starts to come in and know that you do have support. Your baby deserves the best and only you can give it to her.

Your Healthy Newborn - First Days at Home
So you came home with your baby. Whether it is the first-born or not, it's always an exhilarating experience and an opportunity to learn new things everyday.
Your baby probably already had the first Hepatitis B vaccine and the newborn screen at the hospital. Also, he or she already passed the hearing test. Congratulations! This article reviews what you need to know about your newborns developmental milestone, feedings, elimination, and safety.
There are some essential things to know when you bring your neonate home. First of all, per the hospital's recommendations, set up a follow-up appointment with your baby's health care provider (usually 3-4 days post delivery). It is important to evaluate your infant, i.e. check the weight, listen to the heart, etc. Also, you may have questions that need to be answered: prepare a list before the visit so that nothing gets forgotten! Most health care providers like when patients come to the appointment well prepared. It helps us address all of your needs!
Below, I outlined some important things to know about your baby from the very first day.
DEVELOPMENTAL MILESTONES
Your baby should respond to noise. For example, she should startle at the sound of a slamming door or a barking dog. By one month of age, she should be able to distinguish between the mom's voice and voices of other people. However, some babies have this ability from the very first days after birth. Your baby's vision is intact, but right now she can best see objects and faces placed 8-12 inches away from her face. Also, she does not like bright lights. Babies like to open their eyes in dim light by the adult standards.
FEEDINGS
During the first few weeks, don't worry about schedules: your baby should eat on demand, on average every 2-4 hours. However, babies experience rapid growth spurts from time to time, and during those times they may be hungry every 1-2 hours. Just play it by ear. Also, keep in mind that your baby's stomach is very small (about the size of her fist), so it can't hold more than a couple of ounces at the time. If you feed your infant formula and notice excessive spit ups, decrease the amount of the formula but feed your baby a little more often. Don't let more than four hours pass between feedings. You do not want you baby get frustrated with hunger and make the feeding experience unpleasant. At any time, your breastfed or formula fed baby spits up excessively, talk to your health care provider. She may suggest a formula change or, on rare occasions, medication.
ELIMINATION
A healthy newborn should pass the first bowel movement (BM) within 36 hours after being born. After establishing a healthy BM pattern, your baby may go to the bathroom once or a few times a day, or every other day, or so. One thing to keep in minds is that not everybody has a BM every day! As long as you infant is comfortable between BMs and they do not look like pebbles, there is no reason for concern. Also, keep in mind that breast milk has much less waste than formula. Therefore, your baby may have less frequent and malodorous BMs than a formula-fed infant.
Urination is a good indication of hydration. A few days old baby should have at least 4-5 wet diapers and work her way up to more than 6 a day!
SAFETY
The safest sleeping position for your baby is on her back. It decreases the chance of SIDS (Sudden Infant Death Syndrome) (See Box 1). Co-sleeping of infants and parents is no longer recommended. Such sleeping arrangements create an unsafe environment for the baby who can suffocate on bedding or be crushed by a sleeping adult. Every baby should sleep in her own bassinet or crib. Smoking should not be allowed around your baby as well. Cigarette smoke exposure not only increases the risk of SIDS, but it also increases your baby's susceptibility to catching upper respiratory tract infections.
Practice measuring your baby's temperature. Should the temperature be 100F or above, undress the infant and let her cool off. Small babies overheat easily! After 15 minutes, check the temperature again. If the temperature is still 100F or above, your baby needs to be examined by a health care provider right away. That is true until the infant is 3 month old, because until then the immune system does not know how to work efficiently and fever may be a sign of a serious illness.

Box 1. Sudden Infant Death Syndrome (SIDS)
SIDS is a sudden and unpredictable death of a seemingly healthy infant younger than 12 months of age. The diagnosis comes only after other causes of death are ruled out. You can take steps to reduce your baby's risk of SIDS by placing her to sleep on her back even though she might prefer to sleep on her belly! Make sure that the mattress in her crib is firm! Also, offering a pacifier to your infants (only if she does not reject it!) may decrease the risk of SIDS. Avoid cigarette smoke exposure and do not overdress your infant as overheating may contribute to SIDS.

Surviving Bedwetting - A Parents Guide to Bedwetting Treatment and Solutions
The causes of bedwetting in children can be from an imbalance in bladder muscles, a bladder that is too small to hold the amount of urine that they produce, consuming diuretic medication (a substance that directly increases urine output) such as cola or chocolate, making more urine due to a chronic illness such as diabetes, hormone imbalance, or simply from genetics.
Boys are more likely to wet the bed than girls, but girls feel bad about it at an earlier age. Girls are more sensitive to bedwetting early on, but boys eventually catch up. This has to do with the rate at which boys and girls emotionally mature.
While there is no magic wand that you can wave for bedwetting cures, there are some bedwetting solutions you can use to get your child through the tough times. Try one of these effective bedwetting treatments to minimize the effects of bedwetting:
Bedwetting Medication
Your doctor can prescribe a bedwetting medication to help keep your little one stay dry. This medication is a synthetic hormone that helps the body retain fluid through the night. Most people don't want to have their children on medication all of the time, but this could be
an effective option when your child wants to go to a sleepover.
Bedwetting Alarms
A bedwetting alarm has a moisture sensor that attaches to your child's pajamas. When it senses wetness a small alarm will sound alerting the child (and parents) that he or she has wet. If the child hasn't completely emptied their bladder, this may help train them to use the toilet during the night. When you first start using the alarm, someone will need to be there when the child wets to walk him to the bathroom and empty his or her bladder while the alarm is still going off. After a few weeks, the alarm will start waking your child up on their own, and they'll know what to do. After yet a few more weeks, the child will learn to recognize the feeling of having to use the toilet during the night. The good news is that most children don't regress after being trained with this method.
Bedwetting Underwear
Bedwetting Underwear or bedwetting diapers are not effective at keeping your child dry, but they sure will help your sanity and possibly their embarrassment at having wet the bed! With these, you won't have to change the sheets every morning, and your child will not have to sleep in a puddle of urine. Make sure that your child agrees to this method...bedwetting is a very emotional problem, and you don't want them to feel any worse about it than they already do.
Rubber Sheets for Bedwetting
If bedwetting diapers or underwear are not an option, consider using rubber sheets for bedwetting. These will protect your child's mattress, and make an easier clean-up for you in the morning.
I know that it's hard to imagine right now, but this stage of bedwetting will eventually pass. Just remember that bedwetting is usually a normal part of growing up. Your most important job as a parent is to help your young son or daughter make it through this time with as little emotional damage as possible.
